As of May 11, 2026, government hospitals across at least 12 Indian states are reporting stock-outs of 127 medicines from the National List of Essential Medicines. The list includes basic medicines like paracetamol injectable, amoxicillin, ORS, and ferrous sulfate, as well as life-critical drugs such as cisplatin (chemotherapy), regular human insulin, intravenous metronidazole, and pediatric oncology drugs. The shortages have been worst at Tamil Nadu, Maharashtra, Bihar, Uttar Pradesh and West Bengal government hospitals. AIIMS Delhi has reported 18 essential drug stock-outs; AIIMS Patna 31; King George Medical University Lucknow 24. The Indian Pharmacopoeia Commission and CDSCO have confirmed the data. PM Jan Aushadhi outlets have been hit equally hard โ 9,200 of the network's 11,400 stores have one or more essential drugs unavailable. The trigger is a combination of three things: a delayed central procurement tender released six months late; a Karnataka API plant shutdown in March; and Chinese export curbs on three intermediate compounds introduced in February. The Union Health Ministry has called an emergency meeting with state procurement directors; CDSCO is fast-tracking alternative-supplier registration. Patient impact is already documented: a 9-year-old in Patna missed three chemotherapy sessions; pregnant women in three states are unable to access free iron-folic acid tablets.
Four overlapping failures explain the May 2026 stock-outs. First, India's central procurement system for government hospitals is structurally weak. The CMSS (Central Medical Services Society), which procures for central and most state hospitals, runs annual rate contracts that get delayed routinely. The 2025-26 rate contract for 137 essential drugs was floated in October 2024 and was meant to be live by January 2025; it was finalised in March 2025 โ three months late. State-level procurement that depends on CMSS finalisation follows with further delay. Second, API dependence. India is the world's largest generic drug exporter but imports 68% of its bulk drug active pharmaceutical ingredients (APIs) from China, per a 2024 DGCIS report. The 2024 Production Linked Incentive scheme for bulk drugs aimed to bring this share below 30% by 2027 โ current progress is approximately 12%. Third, supplier concentration. For 28 of the 127 short-supply drugs, only two or three manufacturers nationally are qualified to supply central tenders. A single plant shutdown โ the Karnataka API plant in March 2026 โ can disrupt national availability. Fourth, the Jan Aushadhi expansion has out-paced the supply chain. The network has grown from 3,500 stores in 2020 to 11,400 in 2026, but the supply infrastructure has not kept pace. The result is a system that looks robust on paper and is fragile in practice.

Coverage is wide, reliability is not. Myth: API self-reliance is round the corner. Fact: the PLI bulk drug scheme is genuinely shifting the needle, but at current pace will not hit the under-30% import target before 2030.
Nine-year-old Aarav Yadav was diagnosed with acute lymphoblastic leukaemia at AIIMS Patna in November 2025. He responded well to the first two chemotherapy cycles. The third cycle on May 4, 2026 was cancelled because the hospital had been out of cisplatin for nine days. His parents โ both daily-wage labourers in Vaishali district โ sold his mother's gold earrings to buy the drug from a private chemist for 18,000 rupees, money that should not be relevant in a treatment AIIMS officially provides free. They reached for the fourth cycle on May 11. Cisplatin was still out of stock. In a maternity ward at Rajendra Institute of Medical Sciences, Ranchi, pregnant women have been turned away from receiving free iron-folic acid tablets for the past 18 days โ many are anaemic and the prescribed alternatives in the private market cost 280 rupees a month. In Maharashtra's tribal Gadchiroli district, an outbreak of childhood diarrhoea has been complicated by a stock-out of pediatric ORS sachets at five PHCs; three infants required hospital admission for severe dehydration. The thread is consistent: every shortage moves the cost of essential medicine from the central government to the individual household โ and for the poorest, that move is the difference between continuing treatment and abandoning it.
The deeper meaning of the May 2026 shortage is that India has built a world-class pharmaceutical export industry on top of a domestic public-health distribution system that the same industry cannot reliably serve. India exports $25 billion of generic medicines a year โ drugs that save lives in the United States, Africa and Southeast Asia โ while its own government hospitals in Patna, Ranchi and Gadchiroli ration cancer chemotherapy and child ORS. This is not a quality problem; it is a procurement, distribution and political-priority problem. The implications go beyond the current shortage. India spends 2.1% of GDP on public health, one of the lowest ratios in the G20. The lessons of the COVID-era oxygen crisis and the 2026 essential-medicine shortage point to a structural under-investment in domestic distribution capacity. The future of Indian public health requires three commitments: a serious scale-up of public health spending towards 4% of GDP over the next decade; binding accountability for central procurement timelines through public dashboards that show real-time availability; and faster execution of the PLI bulk-drug programme to cut API import dependence below 30% by 2030. The long-term consequence of failing on these dimensions is that every supplier disruption, every Chinese export curb, every API plant outage will produce another shortage like May 2026 โ and each one will be paid for by patients like Aarav Yadav. The shape of Indian public health in the 2030s will be decided by whether the world's pharmacy can finally also serve its own poor.
